AMY JO COOK

Amy Jo Cook, 58, of Harrodsburg, died Thursday, Dec. 17, 2015, in Harrodsburg.

Born March 24, 1957, in Gary, Ind., she was the daughter of the late Donald T. and Shirley May Small Cook.

She was a registered nurse for the VA Hospital in Lexington and was a member of the Cornerstone Assembly of God.

Survivors include: two sons, Jeremy (Ava) Knies of Lexington and Joshua Knies of Harrodsburg; three brothers, Tim Cook of Fla., Tom Cook of SC and Chris Cook of Tenn., and seven grandchildren.

She was preceded in death by one son, Joseph Knies.
